National Family Caregiver Support Program (Title III-E)

The National Family Caregiver Support Program (Title III-E) is a one hundred percent (100%) federally funded program.

There are a variety of services funded through this program:

  • Personal Care
  • Nutrition Counseling
  • Public Information
  • Homemaker
  • Assisted transportation
  • Counseling & Training
  • Chore
  • Transportation
  • Temporary Respite
  • Adult Day Care
  • Information & Assistance
  • Case Management
  • Outreach
  • Medication Management Screening & Education
  • Other (such as Small house modifications; adaptive aids; therapy)

Minimum Requirements:

  1. Information to caregivers about available services
  2. Assistance to caregivers in gaining access to the services
  3. Individual counseling, organization of groups and training to caregivers to assist in making decisions, solving problems relating to their caregiver roles
  4. Respite care to enable caregivers to be temporarily relieved from their caregiving responsibility
  5. Supplemental services, on a limited basis, to complement the care provided by caregivers. These Supplemental services (Personal Nutrition Counseling, Assisted Transportation, Transportation, small home modifications, adaptive aids, professional visits by RN, PT, or OT) are not to exceed 20% of county allocation.
